The short answer is: at least once a year. The National Fire Protection Association (NFPA 211) recommends that chimneys, fireplaces, and vents be inspected at least annually and cleaned as needed. But the right schedule for your home depends on several factors.
Hardwoods like oak, maple, and cherry burn hotter and cleaner than softwoods like pine and fir. Softwoods produce more creosote — the flammable residue that builds up on flue walls — because they burn at lower temperatures and contain more resin. If you primarily burn softwoods, or if you burn green (unseasoned) wood, you may need a sweep twice a year.
Seasoned hardwood burned in a properly sized fireplace with a hot fire produces the least creosote. Even so, annual sweeping is still the standard recommendation.
A household that burns a cord of wood per season has a very different maintenance schedule than one that lights a fire a few times a month. As a general guideline:

Creosote accumulates in stages. Stage 1 is a light, dusty deposit that's easy to remove. Stage 2 is a harder, flakier buildup. Stage 3 is a thick, tar-like glaze that's extremely difficult to remove and highly flammable. Skipping annual sweeps allows Stage 1 buildup to progress to Stage 2 and eventually Stage 3 — at which point you're looking at a significantly higher fire risk and a more expensive cleaning.
Annual sweeping is the baseline. If you burn frequently, use softwoods, or notice any warning signs, schedule more often. The cost of a professional sweep is a fraction of the cost of a chimney fire — or the damage one causes.
